The Physical Therapist is a licensed professional who provides direct and indirect professional therapy care to patients/clients under a physician’s plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Practice Act, and regulatory entities. Duties and Responsibilities: Follow Sunrise Health 21 points of Light and Mission. Follows Agency Policies and Procedures, Federal and State Regulations and Standards of Practice. Attends Agency In-services, especially annual fire, safety, HIPAA, Universal Precautions, and Blood borne pathogens. Attends and participates in Monthly Multidisciplinary Case Conference providing information concerning patients/clients and input into care and discharge planning. Follows Agency procedure of obtaining patients/clients through intake scheduling coordinator. When accepting patient, agrees to make skilled visits to assigned patients/clients as scheduled. Notifies patient/clients prior to visit and communicates further visits      scheduled. Drives responsibly and safely to and from patient’s/client’s home. Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s home. Be responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others.
